Cooking for a new stoma

A new stoma

The first weeks after a stoma is formed, when the opening is swollen and food is kept simple on purpose.

A stoma is an opening brought out through the abdominal wall, with a section of bowel stitched to the skin. For the first weeks after it is formed the stoma itself is swollen, and the opening is therefore narrower than it will settle at. That narrowing is the reason for the food rules of this period, and it is temporary.

What the guidance emphasises

Blockage risk rather than fibre in general. The stoma advice compared for this book is unusually consistent on which foods to keep out at first, and the list is short: nuts, popcorn, mushrooms, sweetcorn, celery and other stringy vegetables, dried fruit, pips and tough skins. Chewing is treated as part of the treatment rather than as manners, because your teeth are the only part of the process you control.

There is a fluid and salt side too. An ileostomy bypasses the colon, which is where most water and salt are reclaimed, so a person with a new ileostomy typically needs more fluid and more added salt than they are used to, and often more than feels correct.

The paths then diverge. A colostomy usually returns to unrestricted, higher-fibre eating after six to eight weeks. An ileostomy tends to keep a longer-term caution around that short blockage list, rather than around fibre generally.

What this book does

Recipes tagged for a new stoma are soft, free of the blockage foods entirely, and easy to cut small. Where a dish might produce wind, the recipe says so. Thickening stoma output covers the starches that firm things up, which is a separate question from fibre.

How long

Weeks. Your stoma nurse and dietitian decide when foods come back and in what order, and the usual method is one new food at a time with a few days between.

If your stoma stops producing output, or if there is vomiting, severe cramping pain, swelling around the stoma or bleeding, that is an urgent call to your stoma nurse or team, not a change of recipe.
