A tall glass of thick, pale, frothy milkshake filled almost to the rim.

Scales by the glass. Blend two portions together in a jug rather than doubling one glass.

This is the drink to reach for when food itself feels like too much to face, because a glass of it does real work. Whole milk gives about 130 kilocalories and 7 grams of protein on its own. Two tablespoons of skimmed milk powder add another 70 kilocalories and 7 grams of protein. A 50 g scoop of ice cream tips in a further 100 kilocalories. Altogether, one glass carries close to 300 kilocalories and 14 grams of protein, more than most people manage from a small meal, in a drink that takes thirty seconds in a blender.

Ingredients

  • 200 mlwhole milk
  • 2 tbspskimmed milk powder
  • 50 gvanilla ice cream
  • 1 tspvanilla extract — optional

Method

  1. Put the milk, milk powder, ice cream and vanilla extract into a blender, or a tall jug if you are using a stick blender.

  2. Blend for 20 to 30 seconds, until completely smooth and slightly frothy on top.

  3. Pour into a tall glass and drink straightaway, while the ice cream is still cold and the texture is at its thickest.

Watch-points

  • If you swap in a flavoured syrup instead of the vanilla extract, choose one without seeds or pieces of fruit running through it, and check the label.
  • A glass this size is mostly milk. Some teams restrict lactose for a while during pelvic radiotherapy or after a low anterior resection, so build it on a lactose-free whole milk if you have been told to.

Stoma note: A drink this rich in milk and sugar can loosen output if you have a lot of it in one sitting. A single glass, drunk slowly, is usually well tolerated.

Make it more nourishing

  • Use a 100 g double scoop of ice cream instead of 50 g, for another 100 kilocalories with no extra volume.
  • Add a third tablespoon of milk powder for another 35 kilocalories and about 3.5 g of protein.
  • Whisk in a tablespoon of double cream for a thicker glass and about 70 kilocalories more.
